PRIOR TO INITIAL CONSULTATION

You will be requested to email a short written medical history prior to your appointment. It should include :

  • symptoms of your current problem

  • timeline of events

  • a list of healthcare professionals attended, treatments received and your responses to those treatments

  • other relevant health history

  • current medications

  • details so the doctor can access your radiology if applicable

REFERRALS

A referral is not required for an appointment at Equilibrium Medicine.

Please note

It is recommended you wear suitable loose clothing (i.e. shorts and singlet) that adequately allows for examination. However, these can be provided if required.

Bringing of a support person with you to your initial consultation is strongly encouraged.

It is not advisable to bring small children to your consultation. Conducting a thorough assessment and formulating a management plan requires your full attention.

Available upon request.

A proportion of Dr. Choong's fees qualify for Medicare rebate, subject to the standard Medicare conditions. Equilibrium Medicine is a private billing clinic.

Private Health Insurance rebates do not apply.

REFERRALS

A referral from your GP is not required for an appointment at Equilibrium Medicine.

CANCELLATIONS

Please call as soon as possible should you wish to cancel. Cancellations within 24 hours of the appointment may attract a cancellation fee.
